The statements herein are derived from the data available at the time of their creation and may need to be modified as new information is generated. The strength of the evidence upon which the statements are based is noted, with prospective, randomized, controlled trials being the strongest. When adequate data are absent, expert consensus may be used and will be identified as such. This document is not to be construed as a standard of care. All decisions regarding the care of a patient should be made by the physician in consideration of all aspects of the patient's specific medical circumstances.
